About the Role:

As a part-time Social Work/Care Companion, you will visit designated facilities, conduct assessments, and implement short-term, solution-focused therapy and supportive interventions to address immediate concerns. Your role is ideal for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment.

Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long‑term care settings
• Strong communication, documentation, and relationship‑building skills
• Education: Associate Degree
• Experience: 1 year
