Job summary

Carease Health is seeking a compassionate Mental Health Provider to join our Collaborative Care team. You’ll visit designated facilities, provide brief therapy, and assess residents in skilled nursing facilities to support their emotional and psychosocial well-being.

Ideal candidates hold an Associates, Bachelor’s or Master’s degree in a mental health field, with at least 1 year of experience and a strong focus on geriatric care.

Qualifications

• Associates, Bachelor’s or Master’s degree in mental health field.
• Understanding of geriatric mental health and crisis management.
• Strong communication, documentation and relationship-building skills.
• Experience: 1 year.

Responsibilities

• Visit designated facilities per supervisor schedule.
• Assess residents to identify emotional, cognitive, psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain records after each visit.
• Collaborate with psychiatric professionals, including monthly meeting with the psychiatric NP.

Carease Health is seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team. In this role, you’ll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

This position is ideal for professionals who want to make a direct impact on the lives of older adults—helping them feel heard, supported, and emotionally connected within their environment.

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.

Qualifications

• Associates, Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or another mental health–related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
• Experience: 1 year
