Job summary

Carease Health in PA is seeking a Social Worker/Care Companion (Part Time) to join our Collaborative Care team. You will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

Visit designated facilities, assess residents’ emotional and psychosocial needs, and implement short-term therapy. Document visit notes and collaborate with psychiatric professionals, including a monthly meeting with the psychiatric NP.

Qualifications

• Degree in social work, psychology, counseling or related field.
• Understanding of geriatric mental health and crisis management.

Job Summary:

As a Social Worker/Care Companion, you will join our Collaborative Care team to provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Social Worker/Care Companion is seeking a compassionate and dedicated Mental Health Provider to make a direct impact on the lives of older adults.

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Education: Associate Degree
• Experience: 1 year
