Job summary

Providence Alaska Medical Center in Anchorage, AK is hiring an Inpatient Social Worker BSW to provide care management for at-risk patients in an acute care setting. This full-time, 1.0 FTE position offers a Day shift (40 hours/week) with a $10,000 hiring bonus for eligible external hires and relocation assistance for qualified candidates.

The BSW Care Manager will coordinate care, plan discharges, document interventions, ensure regulatory compliance and advocate for patients to address barriers

Qualifications

• Bachelor's Degree in Social Work from a CSWE-accredited program is required.
• Alaska Baccalaureate Social Worker License required upon hire.
• Minimum 1 year social work experience in a healthcare setting; recent experience in a medical facility or long-term care setting.

Responsibilities

• Coordinate care, discharge planning, documentation of interventions and patient advocacy.
• Ensure continuity of care for at-risk patients by addressing social, financial, cognitive/behavioral or legal barriers to access.

The Inpatient Bachelor of Social Work (BSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management BSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access.

Required Qualifications:

• Bachelor's Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)

• Upon hire: Alaska Baccalaureate Social Worker License

• 1 year SW experience in any healthcare setting Recent experience in a medical facility or long term care setting.

Preferred Qualifications:

• National Certification in area of specialty

• 1 year experience in acute care setting or successful completion of TIPs program or Case Management Orientation Program

• Basic understanding of acute medical surgical healthcare terminology, prior case management experience.
