HCA Healthcare, Inc. is seeking a Social Worker (BSW) for Medical City Weatherford on a PRN day shift. The role covers inpatient and ED settings, performing non-clinical biopsychosocial assessments, crisis intervention and discharge planning to support patients and families.

The candidate will collaborate with multidisciplinary teams, navigate Medicare/Medicaid and private payer processes, and help arrange post-acute care while honoring patient preferences and values.

Qualifications

• Bachelor's degree in Social Work required.
• Currently licensed as a Registered Bachelor of Social Work in TX.
• Certification in Social Work preferred.
• Licensed Clinical Social Worker preferred.
• Minimum of 1+ years medical social work experience.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurance companies and staff to address patient needs.
• Identify and address discharge and recovery barriers including financial, emotional, psychosocial, medical and spiritual factors.
• Maintain knowledge of Medicare/Medicaid/private payer regulations and managed-care concepts for fiscal responsibility.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate meetings between care team, patients, families and caregivers; support patient preferences and values.
• Intervene for psychosocial needs and provide education, counseling, advocacy and referrals.
• Provide emotional support during end-of-life, crisis situations and complex family circumstances.

Job Summary and Qualifications

* This is a PRN Day Shift position, 8:00am - 4:30pm and will help to cover both weekdays and weekends as needed *

The Social Worker (BSW) is responsible for non-clinical, biopsychosocial assessments and interventions as needed for both the inpatient floors and the outpatient emergency department. The BSW performs crisis intervention, patient/family interventions, high risk screening, focused emotional supports, and referrals for financial assistance or other identified resource needs. The BSW will also assist in post-acute care placement on all discharges, primarily with those patients identified with post-discharge behavioral health and patient assistance/resource needs, and arranges and facilitates Family Care Conferences to support patient, family, and discharge planning goals. The BSW is dedicated to patient and family centered care that values personal self-determination, prioritization, and engaging in creative, compassionate problem-solving. The BSW maintains a thorough working knowledge of the rules of Medicare, Medicaid, and private payer regulations and processes, and possesses a thorough understanding of managed-care concepts and length-of-stay management relating to patient and organizational fiscal responsibility.

The scope of this role includes intensive care units, Trauma, Oncology, NICU, Transplant, ED, and responds to physician, non-discharge planning social work consults throughout the facility.

What you will do in this role:

• You will work collaboratively with RN case managers, acute care nurses, physicians, insurance companies, and other staff members to proactively identify and address patient needs.
• You will identify and address barriers to discharge and recovery. These barriers include financial, emotional, psychosocial, medical, and spiritual.
• You will maintain a thorough working knowledge of the rules of Medicare, Medicaid, and private payer regulations and processes. You will possess a thorough understanding of managed-care concepts and length-of-stay management relating to patient and organizational fiscal responsibility.
• You will perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
• You will coordinate and facilitate meetings between the care team and patients/families/caregivers. You will assist patients and families in making difficult decisions while honoring their preferences and values.
• You will intervene with patients/families/caregivers for psychosocial needs, assistance, and support in coping with chronic acute terminal illness. You will engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
• You will provide emotional support to patients and family members during complex and/or emotionally charged issues, such as end‑of‑life, terminal illness notifications, death notification (with physician), and crisis interventions.

What qualifications you will need:

• Bachelors Degree in Social Work required
• Currently licensed as a Registered Bachelor of Social Work in the State of TX
• Certification in Social Work preferred
• Licensed Clinical Social Worker preferred
• Minimum of 1+ years medical social work experience
