Description

The Social Worker reflects the mission, vision, and values of Northwestern Medicine, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and regulatory standards.

The Social Worker is responsible for providing psychosocial counseling and intervention services to patients, advocating for patients and families, and assisting in outpatient continuity of care.

Desired qualifications: Neurology or Oncology experience.

Responsibilities
  • Complete thorough psychosocial assessments for new patients.
  • Utilize critical thinking to identify and screen high-risk social cases.
  • Provide pre-treatment planning, psychosocial counseling/interventions, health education, financial counseling, referrals, and discharge/transition planning.
  • Collaborate with team members, other departments, case management, and social service providers to develop care plans.
  • Provide resources and education materials to patients and families regarding financial assistance, transportation, scheduling, and community resources.
  • Support department initiatives, attend case conferences, and participate in quality improvement efforts.
  • Perform other duties as assigned.
Qualifications

  • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois.
  • Zero to two years of relevant experience.
