Position Summary
This Social Worker position will be a flexible position to assist with patient and resident needs throughout the Blue Mountain Hospital District. The Hospital District is comprised of Blue Mountain Hospital, Strawberry Wilderness Community Clinic, Blue Mountain Care Center and Blue Mountain Hospice. The Social Worker will work closely with the Hospital Inpatient Case Manager, the Hospice Social Worker and the Care Center Resident Care Manager to meet patient and resident needs.
Participate in performance improvement and continuous quality improvement (CQI) activities.
Duties & Responsibilities
- Promote the Mission, Vision and Values of BMHD.
- Coordinate and collaborate with the inpatient Case Manager on referrals for Swing Bed patients. Participate in multidisciplinary team meetings.
- Assist with discharge planning and possible placement of patients, working with the Case Manager, as needed.
- Provide therapeutic emotional support to patients and their families as needed.
- Facilitate referrals and coordination with other services (e.g. substance abuse treatment, tele-psychiatry, or community resources) as needed or as directed by primary care provider. Develop / maintain a professional relationship and act as a liaison with local mental health agency to enhance coordination of patients being referred for services at their facility.
- Maintain accurate and timely records of activities, case management notes, and services provided to each client.
- Assess resident needs and assist with resident care planning and participate in resident care conferences at the Care Center.
- Help residents and families to fill out Advance Directives.
- Advocate for residents’ needs and ensure rights are protected.
- Participate in required meetings, including multidisciplinary team meetings, quality improvement, or patient centered group sessions.
- Assist in developing policies and procedures specific to mental and behavioral health integrated services.
- Model professionalism and excellent customer service standards and commitment to patient and resident needs.
- Maintain cooperative working relationships with the clinic, hospital staff, patients, residents, other organizations, community partners, and the public to provide quality customer service in a courteous manner.
- Will have working relationship with Hospice Social Worker and ability to assist with home visits if needed.
- Maintain work areas in a clean, orderly and secure manner. Ensure all safety rules and procedures are followed for work areas.
- May assist clinic staff with social work needs.
- Perform other responsibilities or related duties as assigned by the Chief Nursing Officer.
Education Requirements
Required:
- High School Diploma or GED Equivalent
Preferred:
- Experience in critical access hospital setting, long term care, Hospice
- Licensed Clinical Social Worker
Certification & License Requirements
Required:
- Basic Life Support (BLS) certified by American Heart Association (AHA). MUST obtain within 60 days of hire.
- Valid Permit or Driver’s License
