Job summary

Amberwoods of Farmington seeks a Social Worker to join our Case Management team for patients on our short-term rehabilitation unit.

The ideal candidate is patient, positive, organized, proactive and efficient with excellent follow-through. We are building an outstanding team and value enthusiastic, collaborative care.

Qualifications

• Nursing Home or case management experience preferred but can train the right person.
• Bachelor's Degree in Human Services or Social Work is required.

Responsibilities

• Completes required EMR documentation in a timely manner including assessments.
• Completes intake for new admissions including discharge plan.
• Uses resources to set up services for successful discharge after short-term rehab.
• Holds Resident Care Plan Conferences with the interdisciplinary team to review resident's plan of care.
• Assists the interdisciplinary team with any concerns that arise with residents and/or families.
• Supports residents' psychosocial needs.
• Maintains timely and accurate documentation of above.

Amberwoods is a 130-bed beautifully renovated, successful facility providing long-term skilled nursing, short-term rehabilitation and Memory Care services, located in Farmington, CT. Our facility is conveniently located within proximity to I-84 and is a ten-minute drive to downtown Hartford.
