Job description

We are looking for a Remote Social Worker (LMSW) for our AIM (Advanced Illness Management) Program to join our team. You will report directly to the Director of Care Management AIM. You will work with patients, their families, PCP, AIM, and other healthcare professionals to recognize and understand the social/mental stress and/or disorder that magnifies the symptoms related to serious illness and support/educate patients/families in understanding and executing advance care plan documents that are consistent with their wishes. This role will develop and implement plans of care, assess crisis situations to appropriately provide needed resources and referrals for the patient, and provide family and individual interventions as indicated.

Candidate must possess a license in Texas and reside in Texas. This is a remote position but will cover the Plano, TX area.

About You

• Masters of Social Work from an accredited institution
• Able to attain multistate licensing as an LMSW or LCSW
• 2 years of prior social work experience in a healthcare setting; preferably palliative care, advanced illness management, or hospice
• Understanding of palliative care, hospice philosophy, and issues of death/dying
• Knowledge of the impact of terminal diagnosis and anticipatory grief on psychosocial functioning, family dynamics, and community relations, and appropriate therapeutic interventions
• Ability to establish and maintain therapeutic relationships with patients, families, and colleagues
