Job summary

Laurel Circle is seeking a Social Worker to coordinate admissions and discharges and support the Social Services for residents. The role involves working with theHealthcare Team to ensure administrative tasks are completed, maintaining resident rosters, and handling financial updates for private pay residents.

Required is a Bachelor’s degree in Social Work with 1-2 years healthcare experience; two years in a similar social work role is preferred, ideally with long-term care or rehabilitation

Qualifications

• Bachelor’s degree in Social Work required.
• 1-2 years related healthcare experience preferred.
• Two years of long-term care or rehab experience desired.

Responsibilities

• Assist with discharge planning and related administrative tasks.
• Coordinate admissions/discharges with the Healthcare Team.
• Maintain contact with hospitals, doctors and outside agencies for admissions.
• Coordinate forms for admissions and maintain resident roster.
• Update financial records for private pay residents.
• Complete sections of the MDS and participate in training sessions.
• Other duties as assigned.
