A Brief Overview

Collaborates with all members of the health care team, patients, and families (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family-centered discharge plans. Serves as a resource to others in the work team and department.

What You Will Do

• Provides psychosocial assessment and intervention to assist patients and families, including those who are clinically complex.
  
  • Provides age-specific care to patients and families across all age groups.
• Develops, documents, and implements comprehensive plans based on identified psychosocial and discharge needs.
  
  • Develops plans considering individual patient and family goals.
  
  • Provides linkage to community resources and UH network services to facilitate optimal care.
• Communicates and collaborates with the work team using a multidisciplinary approach.
• Provides guidance and consultation to peers and team members regarding routine psychosocial issues and discharge planning.
• Provides supervision to social work interns if LISW eligible, including completion of supervision hours.

Additional Responsibilities

• Performs other duties as assigned.
• Complies with all policies and standards.
• Refer to departmental documentation during orientation for specific duties and responsibilities.
• Must abide by all requirements to safely and securely maintain Protected Health Information (PHI). Annual training, UH Code of Conduct, and policies address appropriate PHI use.
