Job summary

ELMHS in Jackson, LA seeks a Social Worker to deliver client-centered services, including assessment, case management, crisis intervention, and coordination of community resources. The ideal candidate will have licensure in Louisiana (LMSW, LCSW, or CSW) and a master’s degree in social work.

Work Schedule: Monday–Friday. Compensation includes hourly SER with progression through Social Worker 1–4, plus longevity pay; compliance with licensure and permanent-status rules is required.

Qualifications

• Master's degree in social work.
• Louisiana LMSW/LCSW or CSW licensure required; permanent status requires LMSW or LCSW.
• CSW credential not accepted for permanent status.

Responsibilities

• Develop an appropriate discharge and aftercare plan with the patient.
• Document the discharge plan in social assessments and treatment plan updates.
• Locate suitable Permanent Supportive Housing or licensed boarding homes as needed.
• Conduct goal-directed individual and group treatment with adult patients.
• Document active treatment summaries in the patient chart with progress notes.
• Collect and integrate social assessment information from patient, family, and resources within deadlines.
• Participate in Master Treatment Plan development using SMART methodology.
• Participate in mandatory trainings for accreditation/licensure (CPR, CPI, First Aid).
• Maintain a current, valid driver’s license.

The Mission of ELMHS is to provide mental health evaluation, treatment, habilitation, and consultation for adults. Programs include inpatient interventions, jail-based evaluations, supervised group home and residential programs, forensic evaluation, care, custody, and aftercare; intermediate and long-term psychiatric services are coordinated with the total system of care in Louisiana

ELMHS is seeking a passionate Social Worker to deliver client-centered services, including assessment, case management, crisis intervention, and community resource coordination. The ideal candidate will have experience working with diverse populations facing mental health challenges, trauma, or crises.

Examples of duties

Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Certified Social Worker (CSW) plus:
A master's degree in social work.
NOTE:
Permanent status will not be granted in this job title without LMSW or LCSW licensure in Louisiana; Certified Social Worker (CSW) credential will not be accepted for granting permanent status.

Job Duties

In this role, you will:

• Develop an appropriate discharge and aftercare plan in cooperation with the patient
• Document the discharge plan on the social assessments and the treatment plan and update it as needed.
• Locate suitable Permanent Supportive Housing, placements in nursing, group, or licensed boarding homes for those patients unable to return home or require placement.
• Conduct goal-directed and supportive individual and group treatment with affected adult patients
• Document Active treatment summaries on each patient's chart, specifying the patient's progress towards treatment objectives and goals, and response to interventions and reason for continued stay using the BIRP documentation method.
• Collect and integrate social assessment information from patient, family, and other available resources into a complete social assessment within agency-delineated timelines.
• Participate in the development of the Master Treatment Plan using the S.M.A.R.T. method
• Participate in educational trainings that are mandatory for accreditation and licensure of the facility, including but not limited to, CPR, CPI, and First Aid.
• Maintain a current, valid, and unencumbered motor vehicle operator’s license.

  • $29.10 an hour or $2328.00 bi-weekly for Social Worker 4
• This position is eligible for Longevity Pay starting at $2.00 Per Hour Physically Worked
