Job summary

Adoration Health is seeking a Home Health Social Worker to join our Bluefiled, WV team. You will provide expert, patient-centered social work services in home health, guiding families and individuals through crises and care planning.

The role emphasizes independent work with team support, flexible scheduling (PRN), and opportunities for professional growth while delivering compassionate, community-based care for patients in Bluefield, WV.

Qualifications

• Master’s Degree in Social Work required.
• Five plus years of clinical social work experience.
• Valid and current LCSW or LMSW license.

Responsibilities

• Guide and counsel individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them.
• Assist clients in obtaining government funds, education, or treatment.
• Secure information, such as medical, psychological, and social factors contributing to the client’s situation, and evaluate these and the client’s capacities.
• Ask clients to explain problems related to personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments, to determine the nature and degree of the problem.
• Help patients and families through individual or group conferences regarding plans for meeting needs, and aid clients to mobilize inner capacities and environmental resources to improve social functioning.
• Help the client modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and the client’s part in creating them.
• Develop a service plan designed to restore the patient to optimum social and health adjustment within the patient’s capacity

Education

Master’s Degree in Social Work

Five plus years of clinical social work experience

Licensed Clinical Social Worker or Licensed Medical Social Worker
