Social Worker - Skilled Nursing Facility
Vantage at West Springfield | West Springfield, MA

Vantage at West Springfield, a skilled nursing facility, is seeking a qualified and compassionate Social Worker to support the psychosocial needs of residents and their families. This role is an essential part of the interdisciplinary care team and focuses on resident advocacy, care planning, and discharge coordination in a long-term care setting.

The Social Worker is responsible for assessing, planning, and supporting residents’ emotional, social, and psychosocial well-being. This position works closely with nursing, therapy, administration, residents, and families to ensure person-centered care and regulatory compliance.

Responsibilities
  • Complete required social service assessments, including psychosocial assessments and MDS documentation
  • Develop, implement, and update person-centered care plans in collaboration with residents, families, and the interdisciplinary team
  • Support residents adjusting to long-term care, illness, aging, and life transitions
  • Provide counseling and emotional support to residents and families related to grief, loss, and adjustment
  • Address resident concerns and grievances in accordance with facility policies and regulations
  • Participate in discharge planning and coordinate appropriate community resources and services
  • Assist residents and families with advance directives and related documentation (HCP, guardianship, etc.)
  • Maintain accurate, timely, and compliant documentation per state and federal requirements
  • Communicate resident needs and concerns to appropriate leadership
  • Maintain regular contact with residents to promote psychosocial well-being and quality of life
Qualifications
  • Bachelor’s degree in Social Work or related Social Services field required
  • Long-term care or skilled nursing facility experience preferred
  • Knowledge of MDS, care planning, and discharge planning processes
  • Strong communication, organizational, and interpersonal skills
  • Ability to work effectively within an interdisciplinary team
