Job summary

Riverside Health System in Gloucester, Virginia is seeking a Social Worker to support patient and resident needs under supervision of the Director of Social Services or Administrator. The role focuses on discharge and care planning, advocacy, and coordination with the interdisciplinary team to promote the patient’s well-being.

The incumbent will document per Medicare/Medicaid/state requirements, participate in MDS assessments, and respond to resident or family concerns promptly.

Qualifications

• Bachelor's Degree in Social Work or Related Field required.
• Prior work experience in a geriatric or healthcare setting preferred.
• Completion of Administrator In Training (AIT) Program preferred.

Responsibilities

• Documentation completed accurately in accordance with Medicare/Medicaid/State survey regulations and placed on the medical record within the required time frame.
• Provide exposure to LLHARS services that enhance patient quality of life and independence.
• Complete MDS assessments as assigned within the appropriate time frame with minimal corrections.
• Address resident and family concerns/complaints within 24 hours, 95–100% of the time.
• Responsible for discharge planning, care planning, discharge notifications, PASRR forms, Medicaid applications, psychoactive and behavioral monitoring.

It is also the role of the social worker to help promote a smooth working relationship between the Department of Social Services, physician and other disciplines within the facility.
