The Social Worker provides direct clinical social work and case management services to pediatric patients with complex medical needs and their families. Responsibilities include supportive counseling, aiding in adjustment to diagnosis and prognosis, assisting in transition to Blythedale and/or the Steven and Alexandra Cohen Long Term Care Pavilion and coordination of the discharge planning process with a multidisciplinary team. The Social Worker reports to the Director of Social Work or designee.

This is a full-time, onsite position with occasional on-call telephone support evenings, weekends and holidays. Coverage is rotating across the department and averages up to 6 weeks per year.

  • Conduct a thorough psychosocial assessment of the patient, resident, and family at the time of admission and periodically thereafter.
  • Assess patient, resident, and family for specific cultural, spiritual, ethnic, gender-based, and other unique needs and shares this information with the treatment team to promote effective care.
  • Provide supportive counseling as needed to each patient, resident, and/or family.
  • Provide crisis intervention support to each patient, resident, and/or family when needed.
  • Promote patient, resident, and family independence through a collaborative process of advocacy, referrals to community resources and entitlement programs, and supportive encouragement throughout their admission.
  • Provide coverage for other social worker(s) as needed to promote patient, resident, and family support in absence of the assigned worker.
  • Complete documentation in accordance with department standards for each patient and/or resident on assigned caseload and as needed during case coverage for other social worker(s).
  • Identify potential situations of child abuse or neglect and report out to supervisor and/or team; either make any necessary state central registry report directly or is available to support the person making the report.
  • Serve as a liaison between Blythedale, the Steven and Alexandra Cohen Long Term Care Pavilion, and the Mount Pleasant Blythedale union-free school district to promote patient and/or resident education, and with child protection agencies and/or foster care agencies to promote open communication in support of the patient and/or resident.
  • Serve as a point of contact for the patient and/or resident’s insurance provider(s) to promote collaboration between the plan(s) and the treatment team.
  • Participate in department and/or hospital-based teams and perform additional tasks as assigned by the Director or Senior Social Worker.
  • Minimum two years of experience working with children and families and/or pediatric health care is preferred.
  • Hospital or group home experience preferred.
  • Bilingual fluency in Spanish is preferred.
  • Ability to navigate and translate complex care coordination plans effectively to a wide range of stakeholders.
  • Proficiency in Microsoft Office (Word, Outlook, Teams).
  • Familiarity with an EMR (electronic medical record application) preferred.
  • Master’s degree from an accredited social work program.
  • Current New York State LCSW (Licensed Clinical Social Work) or LMSW (Licensed Master Social Work) required.
  • Occasional prolonged standing/walking.
