Job summary

Wakefield Council is recruiting a dedicated social worker for the Transition Team in the Wakefield district. You will carry out person‑led assessments with 16+ service users transitioning from children's services into adult social care, working closely with our partners in the children's teams and the CTLD health team.

The role is a Monday to Friday position, 8:30am–5:00pm (Friday 4:30pm finish). You will contribute to transition planning and ensure the voice of the person guides all decisions.

About the Role:

We are looking to recruit enthusiastic and hardworking, post qualifying social worker within our Transition Team in the Wakefield district.

You will work closely with our key partners in the children's teams and the CTLD health team, to carry out person led assessments with children and adults from the age of 16 years who are transitioning from children's services into adult social care.

The team consists of 5.5 social workers. As a social worker on the team, you will be passionate about making a difference, and ready to embrace change. Working Monday to Friday 8:30am - 5pm and to 4.30pm on a Friday.
