Qualifications

Responsibilities

  • In‑Reach & Baseline Intakes: Conduct initial social assessments and establish transition plans.

  • Care Coordination: Partner with external service providers and align treatment goals with the Clinical Director.
  • Supportive Case Management: Regular 1‑on‑1 progress check-ins to track milestones.
  • Tiered Crisis Support: Serve as Tier‑2 resource during acute emergencies with crisis referrals.

Position Overview
  • Role: Social Worker (Full‑Time Exempt)
  • Location: On‑site — Tallahassee, Florida (with local community navigation)
  • Compensation: $50,000 – $60,000/year + Generous PTO & Diocesan Benefits (Medical, Dental, Vision, HSA with employer contributions, employer‑paid Long‑Term Disability & Life Insurance, and 401(k) retirement plan).
  • Hours: Flexible 40‑hour week. Primary hours incorporate afternoons/early evenings (e.g., 10:00 AM – 6:30 PM, M–F) to align with resident employment and vocational schedules.
  • Reports To: Clinical Director
The Opportunity

We are seeking a compassionate social worker to provide dedicated case management and re‑entry navigation for our intimate residential community of 6 to 8 male residents re‑entering society from state/federal prison (including individuals with non‑violent, violent, and sexual offenses).

Working with a low resident‑to‑staff ratio, you will serve as the primary bridge between clinical therapeutic care and daily real‑world reintegration. You will work within a collaborative team alongside the Clinical Director, a full‑time Health and Wellness Manager, and a full‑time House Manager.

In our tiered crisis framework, the Social Worker provides primary case management and daytime crisis support alongside on‑site residential staff. During off‑business hours, initial on‑site operations and de‑escalation are managed by residential house staff. The Social Worker serves as a Tier‑3 contact for off‑hours in case of emergencies only. The Tier‑2 coordination is provided by the Clinical Director (coordinating crisis referrals, community resources, or care team communications).

Key Responsibilities
  • In‑Reach & Baseline Intakes: Conduct initial social assessments and maintain communication with prospective residents prior to release to establish clear transition plans.
  • Community Re‑Entry Navigation: Assist residents in securing essential documentation (IDs, benefits), transportation, outpatient medical/dental care, educational programs, and employment opportunities.
  • Care Coordination: Partner with external service providers (substance use programs, probation officers, housing networks) and collaborate daily with the Clinical Director to align treatment goals.
  • Supportive Case Management: Conduct regular 1‑on‑1 progress check‑ins with residents to track re‑entry milestones and support daily problem‑solving.
  • Tiered Crisis Support: Serve as a Tier‑2 resource during acute resident emergencies, providing crisis case coordination, emergency referrals, and liaison support alongside on‑duty house staff and the Clinical Director.
  • Documentation & Compliance: Maintain accurate, timely case records and progress notes in compliance with professional standards, confidentiality rules, and organizational policies.
Qualifications
  • Education: Master’s Degree in Social Work (MSW preferred) or Bachelor’s Degree in Social Work (BSW) with strong re‑entry or residential experience. (Open to Registered Clinical Social Worker Interns seeking hours toward LCSW licensure under our licensed Clinical Director).
  • Experience: 2+ years of social work, case management, or residential care experience preferred. Familiarity with trauma‑informed care and criminal justice re‑entry is a strong plus.
  • Mindset: Deep commitment to working with justice‑involved individuals with objectivity, clear professional boundaries, and compassion.
  • Requirements: Valid driver’s license, ability to complete local/regional travel as needed, and successful completion of a standard background check.
