Job summary

Gundersen Health System in Spring Grove, MN is seeking a qualified Social Worker to join our Emplify Health partnership. This full-time role focuses on assessment, planning, education, and referral services for residents and families within the Gundersen network.

You will act as a consultant and leader in comprehensive care across the system, aligning with organizational strategies and goals. A bachelor's degree in social work and MN licensure are required.

Scheduled Weekly Hours: 36

What you will work:

• Full-time: 36 hours/week
• Monday-Friday, Day shifts
• Holiday rotation
• Location: Spring Grove, MN
