Job summary

Genesis Healthcare is seeking a Nursing Center Social Services Specialist to work with patients and families to identify social, emotional, and mental health needs and arrange services to support overall well-being and quality of life.

You will collaborate with the interdisciplinary team, participate in a care plan for each patient, and conduct discharge planning including family education and referrals to community services. A WV Board of Social Work license is required for this role.

Qualifications

• Bachelor's degree in social work or human services field required.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others to provide support and information to improve quality of life.
• Perform discharge planning activities such as education, documentation and referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact with community-based agencies/services that benefit our patient/resident population.

Responsibilities

Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, company policies and procedures, and standards of care.

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well-being. This includes educating staff on the role of social services.


• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.


• Hold meetings with patients/resident, families, and significant others as requested and to provide support and information to improve quality of life.


• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.


• Establish relationships and maintain contact and referral flow with community-based agencies/services that benefit our patient/resident population.
