Genesis Healthcare in Santa Fe, NM is seeking a Social Services Specialist to help identify social, emotional, and mental health needs of patients and coordinate services to enhance well‑being.

You will collaborate with an interdisciplinary team to develop care plans, educate staff on the social services role, and support discharge planning to improve quality of life and ensure compliance with regulations.

Qualifications

• Bachelor's degree in social work or human services field.
• Must possess any certifications/licensures as required by State of employment to practice in long-term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well‑being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community‑based agencies/services that benefit our patient/resident population.

Responsibilities

Enrich lives as a nursing center Social Services Specialist where you will work with patients/residents and their family members/significant others to identify strengths, social, emotional, and mental health needs and then obtain services to meet those needs. The goal is to provide patients/residents with the highest practical level of physical, mental, and psychosocial well‑being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, company policies and procedures, and standards of care.
