Job summary

Intermountain Health Primary Children's Hospital in Salt Lake City, UT seeks a Social Worker to join the outpatient care management team. You will complete psychosocial transplant assessments for Liver Transplant and Dialysis patients, support during clinic visits, and assist with housing, transportation, insurance, and other needs.

A $2,500 sign-on bonus is offered for candidates with 1+ year of experience.

Qualifications

  • MSW from an accredited institution; licensure verification required.
  • Current state licensure (CSW/LCSW) prior to or upon supervision hours.
  • Strong written and verbal communication and critical thinking skills.
  • BLS certification, valid driver’s license, and reliable transportation.

Responsibilities

  • Assessment & Screening: evaluates patients for transition planning, mental health, substance use, and goals of care.
  • Care Coordination: develops and monitors care plans addressing social determinants of health and resources.
  • Behavioral Health Support: uses motivational interviewing and therapy techniques for mental health needs.
  • Therapeutic Intervention: provides brief individual, group, and family therapy and psychosocial assessments.
  • Diagnosis & Referrals: identifies disorders and connects patients to services.
  • Education & Advocacy: trains staff, educates patients, and facilitates safe transitions to the community.
  • Team Collaboration: works with healthcare teams, insurers, and providers for quality care.
  • Quality & Compliance: supports improvement initiatives and policy adherence.
  • Advanced Care Planning: leads goals-of-care discussions with families and teams.

Job Description:This is an exciting opportunity to join our outpatient social work care management team. In this role you will work with patients seen in the Liver Transplant and Dialysis Clinics. As part of this role you will meet with patients and families and complete a psychosocial transplant assessment. You will provide support during outpatient clinic visits as well as before, during, and after the transplant process. Additionally in this role the social worker will assess for behavioral health needs and connect with resources, provide brief social work interventions, identify and troubleshoot any barriers to medical care, complete psychosocial assessments, and assist with needs including housing, transportation, insurance, or other financial concerns. (CSW/LCSW)Basic computer proficiency, including familiarity with word processing and spreadsheet software.Strong written and verbal communication skills.Demonstrated ability to apply critical thinking skills.Caregivers whose duties require them to conduct home or community visits must maintain current BLS certification, have a current driver’s license, current auto insurance, an acceptable driving record and reliable transportation.Preferred QualificationsCase Management Certification.Experience in clinical care management, social work, or working with third-party payers.Demonstrated understanding of care management principles and practices.Demonstrated understanding of health insurance products and related processes.Demonstrated understanding of coding, episode of care, and length of stay guidelines.Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment.Physical RequirementsOngoing need for employee to see and read information, documents, monitors, identify equipment and supplies, and be able to assess customer needs.Frequent interactions with providers, colleagues, customers, patients/clients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy.
