Park Nicollet Methodist Hospital is seeking a Social Worker/Care Manager to join our team as a partner for good. The role focuses on patient-centered care management within an inpatient hospital setting, coordinating with care teams and patient families to ensure effective transitions.

The position emphasizes collaboration across disciplines, adherence to social work values, and ongoing quality improvement in daily practice. FTE .8 with weekday schedule and rotating weekends.

Qualifications

• BSW (Bachelor's of Social Work) required or equivalent.
• Current Minnesota license to practice as LSW.

The Social Work Care Manager provides patient-centered care management services to patients and families in an inpatient hospital setting. This position serves as an integral member of the interdisciplinary healthcare team and is responsible for identifying care coordination needs in collaboration with the inpatient and ambulatory care teams, along with the patient and the patient's external support systems.

Within the context of an interdisciplinary team, the Social Work Care Manager:

• Participates in progressing patients toward discharge/next level of care by identifying barriers, implementing appropriate interventions.
• Securing transition plans that are aligned with shared goals of care.
• The Social Work Care Manager will consistently and thoughtfully apply social work values and continuous quality improvement in daily work.

Schedule: FTE .8 (32 hrs. weekly) 8 block schedule between M-F 8a-4:30p and every 4th weekend rotation.

REQUIRED QUALIFICATIONS:

• Education, Experience or Equivalent Combination: BSW - Bachelor's of Social Work
• Licensure/ Registration/ Certification: Current Minnesota license to practice as LSW
