Job summary

Park Nicollet Methodist Hospital seeks a Social Worker/Care Manager to join our team, providing patient-centered care management in an inpatient setting.

You will collaborate with inpatient and ambulatory teams, identify discharge barriers, and secure transition plans aligned with goals of care while upholding social work values and quality improvement in daily practice. Schedule: 32 hrs/week, M-F with rotating weekends.

Qualifications

• Degree required: BSW (Bachelor's of Social Work).
• Current Minnesota LSW licensure required.
• Inpatient hospital setting experience preferred.

Responsibilities

• Identify care coordination needs with inpatient/ambulatory teams and patients.
• Develop transition plans aligned with goals of care.
• Apply social work values and continuous quality improvement in daily practice.
