Job summary

Cynet HealthStaff is seeking a Social Worker to assess and intervene for inpatients, outpatients, and ED patients within a hospital setting. You will develop treatment plans and coordinate with physicians, case managers, and community resources to support patient-centered care.

The role requires an MSW, a minimum of two years in clinical/medical social work, BLS and 5150 certifications, and recent experience in a similar setting.

Qualifications

Responsibilities

• Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients.

• Collaborate with physicians, case managers, nursing staff, county agencies, and community resources.
• Conduct crisis intervention, brief counseling, and high-risk screening.
• Arrange and facilitate family/patient meetings with the healthcare team.
• Assist in post-acute placement on complex discharges and coordinate with agencies.
• Participate in hospital committees and staff in-services; pursue county 5150 certification.

Description:The Social Worker is responsible for performing social work assessments and interventions as needed for inpatients, outpatients, and emergency department patients.They are dedicated to patient and family-centered care that values personal self-determination.They are skilled at managing multiple priorities at once and engaging in creative, compassionate, and ethical problem-solving.As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate medical, social, and resource issues impacting individual patients, families, and at-risk populations.Social Workers are responsible for working collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered care for both individual patients and the community.The functions of the Social Worker include crisis intervention, patient and family intervention, high-risk screening, brief counseling, and referring for financial or other identified resource needs.They arrange and facilitate family/patient representative meetings with the healthcare team as needed.They assist in post-acute placement on complex discharges and engage with appropriate agencies or community resources when high-risk patients are identified.The Social Worker establishes and maintains professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.The position will require serving on hospital committees and participating in developing and presenting in-services to staff.The Social Worker will need to become 5150 certified at the county level.External hires must pass a post-offer, pre-employment background check/drug screen.
