Job summary

Vernovis is seeking a Social Worker to provide social work services to diverse patient groups affected by illness or disability. The role requires understanding case management principles, community resources, and regulatory knowledge to support patients and families.

Minimum MSW with at least two years of related experience; healthcare experience and licenses/certifications such as BLS are preferred, with mandatory de-escalation training within 3 months of hire.

Qualifications

  • Master's degree in Social Work (MSW) required.
  • Two years of social work and medical crisis intervention experience preferred.
  • Healthcare setting experience preferred.
  • Experience in individual and family therapy preferred.
  • BLS Certification and de-escalation/restraint certification required within 3 months of hire.

Responsibilities

  • Provide social work services to all customer groups affected by illness or disability.
  • Understand utilization criteria and case management principles in a healthcare setting.
  • Coordinate with community resources, insurance, regulatory, and legal systems.
  • Offer psychosocial assessments and support to patients and families.
  • Facilitate individual, family, and group interventions as needed.
  • Participate in rotational coverage as applicable.

Understands utilization criteria and case management principles in a health care setting, as well as relevant community resources, insurance, regulatory, and legal system knowledge.

Minimum Education
  • Master's degree in Social Work (MSW)
Minimum Work Experience
  • Two years of social work and medical crisis intervention, and knowledge of community resources or related experience preferred.
  • Healthcare experience a plus
  • Experience in individual and family therapy.
  • Five years' experience working in a healthcare setting preferred.
Required Licenses/Certifications
  • BLS Certification through American Heart Association*
  • Completion of endorsed patient/visitor de-escalation and restraint program certification *
  • Eligible for rostering in preparation for licensure.

*Must obtain within 3 months of hire

  • Psychosocial assessments, utilization and case management principles.
  • Demonstrates sound knowledge of group theory and dynamics with experience in facilitating and co-facilitating psychosocial and skill building groups.

  • Demonstrates excellent collaboration skills in conjunction with working with providers, legal representation, regulatory agencies and those involved with risk and compliance.
  • Willingness and ability to provide coverage on rotational basis.(as applicable)
  • Exhibits knowledge of Best Practice, DBT, CBT, and motivational interviewing.
  • Provides employee assistance in-services, and consultation to staff, physicians, and community providers about psychosocial aspects of care.
