Job summary

Winning Wheels, Inc. is seeking a salaried Social Worker to develop and review care plans addressing residents' psychosocial needs in a Illinois facility. You will complete quarterly assessments, handle MDS portions, and maintain open communication with residents' families.

The role involves discharge planning, coordinating resources and referrals, and performing administrative duties. Normal Monday–Friday with rotating Manager on Duty weekends.

Qualifications

• Must have a bachelor's degree, or equivalent in social work or related field, or be a Licensed Social Worker.

Responsibilities and Duties

Develop, implement, and review care plans designed to meet the psychosocial needs of the resident. Complete quarterly assessments, and necessary MDS portions as well.

Maintain ongoing communication with residents’ families, to ensure families are aware of changes, updates, etc., regarding their loved ones care.

Participate in discharge planning, and navigating resources/referrals appropriate and needed for the resident.

Perform administrative requirements, such as completing daily documentation, necessary forms, reports, etc., and submitting such to the Director as required.

This is a salaried position.

M-F with rotating Manager on Duty Weekends

Qualifications and Skills

Must have a bachelor's degree, or equivalent in social work or related field, or be a Licensed Social Worker.
