Nursing Home Social Worker

Richmond Center for Rehabilitation and Healthcare is seeking a full-time Social Worker for our Skilled Nursing Facility in Staten Island, NY. The ideal candidate will have excellent communication skills and a pleasant demeanor.

Prior long-term care experience required.

Duties Include:

• Maintaining and/or improving residents’ physical, mental, and psychosocial well-being
• Identifying individual social and emotional needs
• Developing and maintaining care plans
• Participating in admission, re-entry, or transfer planning
• Participating in interdisciplinary care plan meetings to address social and emotional needs

Requirements:

• LMSW preferred
• Prior long-term care/sub-acute experience required
