Job summary

MJHS is a large not-for-profit health system serving the Greater New York area, delivering hospice and palliative care with a strong commitment to patients and families. This role provides psychosocial support to hospice patients and families in inpatient units, nursing homes, or home care settings, with emphasis on bereavement and care planning.

Graduate social workers with NY MSW licensure are encouraged to apply.

Qualifications

Graduate from an accredited School of Social Work.
Minimum of one year experience in Social Work in a healthcare setting.
Knowledge of Word and Excel preferred.
New York State MSW licensure.
Ability to work within an interdisciplinary team environment.
Possess excellent organizational, communication and listening skills.
Ability to input clinical data proficiently.
Car and driver license required.

Responsibilities

Provide psychosocial support and Social Work services to Hospice patients and families.
Provide pre and post bereavement services.
Serve as Social Work member of hospice IDG and liaison with staff.
Develop social work plan of care and set goals.
Maintain accurate documentation of assessments and interventions.
Participate in discharge planning when appropriate.
