You will collaborate with nursing, therapy, and medical teams to facilitate discharge planning, advocate for residents’ rights, and ensure accurate documentation. This role emphasizes empathy, professional communication, and coordinated care to enhance quality of life.

Qualifications

• BSW required; MSW preferred.
• Experience in skilled nursing or healthcare setting a plus.

Green Hills Center for Rehabilitation and Healing is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. In this role, you will play a vital part in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and enhancing overall quality of life within our skilled nursing environment.
