Job summary

HCA Healthcare's Centennial Heart in Nashville is seeking a Licensed Social Worker to evaluate and serve patients across age groups, collaborating with the interdisciplinary team to develop and monitor treatment plans and document care in the clinical record.

This full-time day position requires 3-5 years of experience and a Clinical Mental Health Counselor or Independent Clinical Social Worker license, with strong communication to patients and families.

Qualifications

• Post Graduate degree required.
• Clinical Mental Health Counselor license or Independent Clinical Social Worker license required per state regulations.

Responsibilities

• Performs vocational rehabilitation assessment and documents report in the clinical record.
• Initiates and discharges documents within required timeframes.
• Documents therapy notes in the patient record.
• Forms and reviews initial and comprehensive treatment plans.
• Identifies and documents patient care needs.
• Collaborates with the interdisciplinary team to ensure appropriate patient care and maintains patient rights.
• Provides patient/family emotional support and education.
• Meets in-service and mandatory education requirements.
• Performs other duties per Physician Services policies and procedures.

Job Summary And Qualifications

Full time (Monday - Friday, days), no nights, no on-call, no weekends, no holidays

Seeking a Licensed Social Worker who performs evaluations and services to patients of various age groups. Collects data and renders a professional opinion about treatment and care.

What will you do in this role:

• Performs vocational rehabilitation assessment in a timely manner; dictates interdisciplinary report to clinical record within the required time frames.
• Initiates and discharge documents within the required timeframe.
• Documents of therapy notes on the clinical record.
• Formulates and reviews initial and comprehensive treatment plans within the required time frame.
• Identifies and documents patient care needs.
• Works closely with the interdisciplinary team to ensure appropriate patient care. Knows, understands, maintains, and monitors patients’ rights.
• Provides patient/family emotional support and education
• Meets in-service requirements including mandatory education
• Perform other duties as required. Must read, understand, and adhere to all Physician Services policies and procedures.
• Practice and adherence to the Code of Conduct and Mission and Value Statement.
