Appledore Family Med GME seeks a Licensed Social Worker for full-time day shifts in Portsmouth, NH. The role involves evaluating patients across age groups, performing timely assessments, and coordinating care with the interdisciplinary team.

Responsibilities include treatment planning, documentation, and providing emotional support to patients and families. Requires 3–5 years of experience and a state-licensed Clinical Mental Health Counselor or Independent Clinical Social Worker license.

Qualifications

• Licensed Social Worker with required licensure per state regulations.
• Experience in performing evaluations and services across age groups.
• Strong data collection and documentation skills.

• Initiates and discharges documents within required time frames.
• Documents therapy notes in the clinical record.

• Provides patient/family emotional support and education.
• Meets in-service requirements including mandatory education.
• Performs other duties as required per Physician Services policies.

Job Summary And Qualifications

Full time (Monday - Friday, days), no nights, no on-call, no weekends, no holidays

Seeking a Licensed Social Worker who performs evaluations and services to patients of various age groups. Collects data and renders a professional opinion about treatment and care.

What will you do in this role:

• Performs vocational rehabilitation assessment in a timely manner; dictates interdisciplinary report to clinical record within the required time frames.
• Initiates and discharge documents within the required timeframe.
• Documents of therapy notes on the clinical record.
• Formulates and reviews initial and comprehensive treatment plans within the required time frame.
• Identifies and documents patient care needs.
• Works closely with the interdisciplinary team to ensure appropriate patient care. Knows, understands, maintains, and monitors patients’ rights.
• Provides patient/family emotional support and education
• Meets in-service requirements including mandatory education
• Perform other duties as required. Must read, understand, and adhere to all Physician Services policies and procedures.
• Practice and adherence to the Code of Conduct and Mission and Value Statement.

EXPERIENCE:

• Minimum Of 3-5 Years’ Experience

CERTIFICATION/LICENSE: 3 ways to qualify:

• A Clinical Mental Health Counselor license or Independent Clinical Social Worker license is required in accordance with state regulations.
