Caris Healthcare, LP in Morristown, TN is seeking a Social Worker to join our hospice interdisciplinary team. You will perform comprehensive psychosocial assessments, identify care planning goals, and provide emotional support to patients and families at end of life.

Requirements include MSW preferred or BSW, at least 1 year of health care social work experience, and strong communication skills. The position offers benefits, mileage reimbursement, and flexible scheduling in a compassionate team

Formación

• TN and MO = MSW preferred; BSW required.
• 1 year social work experience in a health care setting.

Responsabilidades

• Completes and documents psychosocial assessments in EMR.
• Identifies care planning goals and develops interventions.
• Participates in Interdisciplinary Team meetings; provides psychosocial insight.
• Provides education related to funeral planning and advanced directives.
• Conducts visits with patients and caregivers in their homes or residence.
• Assists with bereavement support for caregivers after patient death.
• On-call rotation for after-hours visits as needed.

Position: Social Worker

Pay: $58000 - $65000 yearly Depending on Degree and Experience

Caris Healthcare Social Worker provides support the hospice interdisciplinary team through office functions. Responsibilities

• Completes and documents comprehensive psychosocial assessments with patients in the electronic medical record
• Identifies care planning goals and develops interventions to promote best outcomes
• Participates in Interdisciplinary Team meetings; provides insight into the psychosocial and emotional needs of patients
• Provides education related to funeral planning and advanced directives as needed
• Conducts visits with the patient and caregiver in their residential setting
• Assists with bereavement support and interventions for caregivers following the death of a patient
• Participates in an on-call rotation to make after hours visits when needed for a patient’s death, funeral, admission, etc.

Qualifications

• Tennessee and Missouri = MSW preferred, BSW required
• South Carolina = Social Work Licensure Required; LMSW required
• One (1) year of social work experience in a health care setting
• Excellent communication and interpersonal skills
