Park Nicollet Health Services is seeking a Social Worker to join the Minneapolis Primary Care team. The role delivers psychosocial support to patients and families, coordinating care with hospital and clinic social workers and other providers. Onsite, with a standard weekday schedule.

Responsibilities include direct client services, coordination across departments, and potential supervision of other social workers. Licensure in Minnesota (LICSW or LISW) is required for this full-time position.

Qualifications

• Master’s degree in social work required.
• Licensed as LICSW or LISW in Minnesota.
• Current license to practice social work in Minnesota.

Position Summary:

Provides direct social work services to patients, extending services to family members in need of identified social worker services, providing them with psychosocial and environmental support. Makes available a full range of treatment modalities with individuals, families, and groups, based on social work assessment and diagnosis. Coordinates social work services with other departments and providers within the Park Nicollet Health Services i.e., hospice, hospital and clinic based social workers and out of system providers. Provides social work services to the patient and associated community. May provide clinical supervision to other Social Workers. Works with physicians, nurse clinicians, other medical/surgical specialists, and other Park Nicollet Health Services employees. Contact with patients and their families.

Work Schedule:

• Monday-Friday
• 8:00 AM-5:00 PM; occasional schedule variations based on business needs.
• Position is fully onsite and not eligible for remote work.

Required Qualifications:

• Master’s degree in social work.
• Must be licensed as Licensed Independent Clinical Social Worker (LICSW) or LIS
• Current license to practice social work in the state of Minnesota.
