Job summary

Athena Health Care Systems is hiring a Social Worker to join the care team at Wadsworth Glen Health Care and Rehabilitation Center in Middletown. The role works 32 hours per week, 8a-4p, with compensation of $26 to $30 per hour and weekly pay.

The social worker will plan, organize, develop, and direct social services to meet residents’ need and ensure emotional and social needs are met on an individual basis. Minimum one year in a related setting and a BSWS preferred.

Qualifications

• Minimum one year of social work experience in health care settings preferred.
• Bachelor’s degree in Social Work from an approved school of social work.

Responsibilities

• Coordinate and monitor resident care conferences.
• Work with residents, families, and responsible parties toward adjustment to placement and crisis.
• Protect residents’ health information per Privacy Practices and policies.
• Complete comprehensive documentation in a timely manner.
• Advocate for residents’ personal needs.
• Utilize community resources appropriately.
• Support staff regarding resident-related issues.

We are hiring a Social Worker to join our dynamic care team at Wadsworth Glen Health Care and Rehabilitation Center in Middletown!

32 hours a week. 8a-4p schedule. Wadsworth Glen is a 102-bed skilled nursing facility. $26 to $30 an hour with weekly pay.

Your role is to plan, organize, develop, and direct all required social services to meet residents' needs and ensure that residents' medically related emotional and social needs are met/maintained on an individual basis.

Experience & Education:

• A minimum of one (1) year social work in a hospital, skilled facility, or other health-related field preferred.
• Must possess, as a minimum, a bachelor’s in social work from an approved school of social work.

Duties & Responsibilities:

• Coordinate and monitor resident care conferences.
• Work with individual residents, responsible parties, and families towards adjustment to placement and to crisis (i.e., illness, disability, and death).
• Complies with facility Privacy Practices policies and procedures and protects residents’ individual health information.
• Complete comprehensive documentation in a timely manner.
• In the role of advocate coordinate personal needs.
• Utilize community resources appropriately.
• Support staff in relation to resident-related issues.
