Home Caregivers Partnership seeks a qualified Social Worker to provide patient-centered support in home health settings. You will focus on social and emotional factors influencing health outcomes and collaborate on care plans with the clinical team.
The role requires handling Plan of Care development, documentation, and community referrals, with responsibilities including education and family support during discharge planning and bereavement counseling when needed.

Provide qualified support and care to patient/family in home health services, focusing on the resource management and issues directly related to the purpose the patient is on home health, and issues effecting the goals of treatment.
When medical social services are provided, they shall be provided by a certified Social Worker (CSW) or by a social service worker (SSW) supervised by a certified Social Worker, in accordance with the Plan of Care. All social workers (LCSW, CSW, or BSW) report directly to the Clinical Manager.
The Social Worker shall be responsible to:
Works indoors in the Agency office and patient homes and travels to/from patient homes.

Bachelor of Social Work Degree from a school of social work accredited by the Council of Social Worker Education required. Master of Social Work preferred.
Licensure in the State of Utah under the Mental Health Professional Practice Act (Title 58, Chapter 60).
Two years of experience in medical social work.
Working knowledge of grief process, ethical issues.
