Qualifications

• Bachelor’s degree in Social Work (BSW) or related field; Master’s degree preferred but not required.
• Valid state licensure or certification as a Licensed Social Worker (LSW) or equivalent.
• Proven experience working in healthcare or skilled nursing environment.
• Strong interpersonal and communication skills, with the ability to build rapport with diverse populations.
• Knowledge of healthcare regulations, Medicaid/Medicare policies, and community resources.
• Ability to handle sensitive situations with empathy, professionalism, and discretion.
• Excellent organizational and documentation skills.
• Ability to work collaboratively within a multidisciplinary team.
