Job summary

Capitol Rehabilitation and Healthcare Center in Harrisburg, PA, is hiring a Social Worker for a full-time role. You will assess psychosocial needs, develop care plans, and coordinate services to improve residents’ quality of life in a 138-bed sub-acute and long-term care setting.

The position emphasizes collaboration with residents, families, and interdisciplinary teams, with duties including counseling, care planning, and ensuring regulatory compliance in a fast-paced environment.

Qualifications

• Bachelor’s degree required; Master’s degree strongly preferred.
• Minimum of 1+ years of experience as a Social Worker in a healthcare setting.
• Experience in a nursing home or rehabilitation center strongly preferred.
• Strong knowledge of regulations governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, staff and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.

• Ensure compliance with all applicable facility policies, regulations, and documentation standards.
