Job summary

The Judge Group is seeking a qualified Social Worker for a Care Management role in Garden Grove, CA, with a Monday–Friday schedule and an assignment through October 31, 2026. The position emphasizes interdisciplinary collaboration to resolve psychosocial needs, coordinate services, and promote independence.

Responsibilities include leading care planning, liaison with families and providers, resource development, documentation, and proactive member support.

Qualifications

  • Master’s degree in Social Work.
  • 2 years of experience working with gerontology populations.
  • Experience in behavioral health, health facilities, health plans, counseling, case management, home health, or hospice.
  • Equivalent combinations of education and experience may be considered.
  • Valid driver’s license, reliable transportation, acceptable driving record, and auto insurance (required for roles with 50%+ travel).

  • Initiates follow‑up care and develops individualized care plans (assessment, education, coordination).
  • Accepts case management referrals and serves as a liaison to community agencies and State of California personnel.
  • Maintains documentation and statistics demonstrating case management impact.
  • Initiates case conferences as needed.

Work Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.

Assignment Duration: Through October 31, 2026 (subject to adjustment)

Responsibilities

Support Services — 25%
  • Contributes to a mission‑driven culture emphasizing quality, customer service, consistency, dignity, and accountability.
  • Supports departmental responsibilities and organizational goals.
  • Develops and maintains a network of community resources and services.
Other Duties — 5%
  • Completes additional projects and responsibilities as assigned.
Minimum Qualifications
  • Master’s degree in Social Work.
  • 2 years of experience working with gerontology populations.

Preferred Qualifications
  • Bilingual in English and one of the following: Arabic, Farsi, Chinese, Korean, Russian, Spanish, or Vietnamese.
