Responsibilities

  • Plan, organize, and supervise social services programs and staff.
  • Ensure timely completion of resident assessments and required documentation.
  • Ensure compliance with PASRR and applicable federal and state regulations.
  • Assign, review, and evaluate the work of Social Services staff.
  • Provide coaching, performance feedback, and staff supervision.
  • Assist with interviewing, selecting, and training staff.
  • Coordinate safe and appropriate discharge planning with residents, families, and healthcare providers.
  • Monitor required Medicare notices and documentation.
  • Coordinate referrals and consultations with healthcare specialists and community resources.
  • Advocate for resident rights and assist residents and families with social service needs.
  • Maintain a personal caseload and provide case management services.
  • Assist with department budgeting and monitor expenditures.
  • Participate in interdisciplinary meetings, committees, and quality improvement activities.
