Job summary

Heart to Heart Hospice in Conroe, TX seeks a Social Worker to assess psychosocial needs of patients and families, provide counseling, and connect them with community resources.

Requirements: Bachelor’s or Master’s in social work from an accredited program, current state license, valid driver’s license with reliable transportation, and at least one year of clinical social work experience (hospice preferred).

Qualifications

• Bachelor's or Master’s degree in social work from an accredited program.
• Current state license required in accordance with state regulations.
• Current Driver's License with reliable transportation and active auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
