Job summary

Sans Souci Rehabilitation and Nursing Center is seeking a qualified Social Worker to join our team in Yonkers, NY. The role involves coordinating medical related social services, identifying resident needs, and developing care plans in collaboration with an interdisciplinary team.

The successful candidate will have 1+ year experience in social work, preferably in long-term care or sub-acute settings, with flexibility in scheduling and knowledge of MDS 3.0.

Qualifications

• BSW or MSW preferred, or Bachelor's degree in related field.
• At least one year experience in Social Work field required.
• Long term care and/or sub-acute setting preferred.
• Ability to work flexible schedule.
• PCC experience preferred.
• Knowledge of MDS 3.0 preferred.

• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community
