Job summary

You will conduct comprehensive assessments to understand needs, strengths, and goals; develop and implement individualized care plans in collaboration with clients and their support networks; maintain accurate, confidential records in compliance with standards;

Qualifications

• Bachelor’s or master’s degree in social work (BSW/MSW) from an accredited program.
