Job Description:

The Social Work Care Manager collaborates with physicians and healthcare team members to improve patient health outcomes, especially for those with chronic or complex conditions. Responsibilities include educating patients and caregivers, facilitating communication, coordinating services, addressing barriers, and promoting efficient resource use while balancing quality and cost management.

Job Specifics

  • Pay Range Clinical: $34.06-$52.55 Non Exempt
  • Pay Range Certified: $30.94-$47.77 Non Exempt
  • Benefits Eligible: Yes
  • FTE: Part time
  • Shift: Days, 0800-1630, with every other weekend requirement
  • Fully remote position

Job Essentials

  • Identify patient populations using screening criteria, medical record review, or referrals, including trigger lists and pre-screen criteria.
  • Review medical records and gather relevant data through interviews.
  • Develop and maintain care plans per CMS COP or NCQA standards.
  • Coordinate and facilitate communication among patients, caregivers, healthcare team, and external parties.
  • Ensure timely and appropriate sequencing of tests, procedures, and consults.
  • Intervene to resolve barriers across the care continuum and maintain awareness of community resources.
  • Perform cost-benefit analyses and coordinate authorizations for care levels.
  • Diagnose mental, emotional, and behavioral disorders using DSM-5 guidelines and provide appropriate interventions.
  • Collaborate with external agencies on issues of abuse, neglect, or exploitation, acting within legal scopes.

Minimum Qualifications

  • Master's Degree in Social Work from an accredited institution (verification required).
  • Current State Licensure as a CSW or LCSW.
  • Basic computer skills, including word processing and spreadsheets.
  • Strong communication and critical thinking skills.

Preferred Qualifications

  • Case Management Certification.
  • Experience in clinical care management, social work, or third-party payers.
  • Knowledge of care management, health insurance products, coding, and care guidelines.
  • Independent, motivated, adaptable, and positive attitude in a dynamic environment.

Physical Requirements:

  • Communication with others.
  • Operate computers and office equipment.
  • Read monitors and documents.
  • Remain seated or standing for extended periods.

Location: Valley Center Tower, Murray, Utah

Scheduled Weekly Hours: 24

The hourly rate varies based on experience, ranging from $34.06 to $52.55.
