Job summary

Qualifications

• Complete psychosocial assessments and identify residents’ emotional, social, and psychological needs.
• Develop and implement individualized care interventions that support resident goals and quality of life.
• Provide support and guidance to residents as they adjust to the nursing home environment.
• Advocate for resident rights, dignity, independence, and personal choices.
• Provide emotional support during challenging transitions and life events.
• Assist residents and families with accessing appropriate community resources and services.
• Participate as an active member of the interdisciplinary care team.
• Collaborate with nursing, rehabilitation, dietary, administration, and other departments to support resident-centered care.
• Participate in care planning meetings and quality improvement initiatives.
• Assist with discharge planning and coordination of appropriate community services.
• Provide education and support to residents, families, and caregivers regarding available resources and services.
• Maintain accurate and timely documentation related to resident assessments, interventions, and progress.
• Ensure compliance with applicable state and federal regulations and professional standards.
• Support facility initiatives related to resident rights, quality improvement, and person-centered care.
• Maintain confidentiality of resident information in accordance with HIPAA regulations.
