Sturdy Memorial Hospital in Attleboro, MA seeks a Social Worker to evaluate patients’ needs and to coordinate discharge planning. You will develop and update discharge plans based on initial assessments and ongoing reassessments, communicating with doctors, nurses, home health providers, and other care professionals to ensure smooth transitions.
The role requires a MA LCSW license (LICSW preferred) and 3–5 years in an acute care setting.

The Social Worker is responsible for evaluating the mental, emotional and physical status of patients, while managing care coordination needs in developing and executing the discharge plan. The Social Worker is responsible for creating a discharge plan based on an initial assessment, with ongoing reassessments with modifications as required. Communicating with doctors, nurses, home health companies and other healthcare personnel and agencies

Salary Range: $31.87-$49.13
