Job summary

AltaMed Health Services seeks a Social Work Supervisor for our PACE program in the Los Angeles area. The role includes supervising social work staff, coordinating assessments and care planning, and guiding team documentation and new-hire onboarding.

Qualified candidates will hold an MSW, have at least two years of supervisory experience in health or human services, and demonstrate bilingual proficiency. A strong knowledge of social work regulations and life-planning guidance is essential.

Qualifications

• Master's degree in social work (MSW) required.
• LCSW preferred.
• Minimum 2 years of supervisory experience in health-related or human services fields.
• Minimum 2 years of case management/social advocacy/care planning with geriatric population preferred.
• Bilingual Spanish/English, Chinese/English, or another second language strongly preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide administrative oversight and supervision of direct reports at the assigned PACE site.
• Coordinate routine assessments and care plans; assist with family conferences and community referrals.
• Monitor team documentation and onboarding/orientation of new hires; conduct competency and performance evaluations.
• Provide guidance on behavioral health, life planning and end-of-life care within regulatory frameworks.

Promotes effective collaboration and individualized care planning amongst interdisciplinary team members. Responsibilities include but are not limited to the coordination of routine assessments and care plans, assisting with family conferences and community referrals, monitoring of team member documentation, onboarding and orientation of new hires, as well as routine competency and work performance evaluations. Provides guidance and support related to participants’ behavioral health concerns to ensure a person-centered approach, and provides guidance related to advanced life planning and end-of-life care. Utilizes knowledge of state and federal regulatory requirements related to social work department tasks to provide effective oversight and ensure compliance. Collaboration with other site leaders to monitor site safety, overall staffing changes, and program budgets. May provide coverage of SW department roles as needed.Minimum RequirementsA master's degree in social work (MSW) from an accredited university is required.
