Job summary

Centers Health Care in Staten Island, NY is seeking an experienced Director of Social Work (DSW) to lead our Social Services Department within a premier rehabilitation and skilled nursing facility. You will oversee resident assessments, care planning, and transitions, while guiding a team of social workers.

This role requires a licensed MSW/LSW and at least 2 years of supervisory experience in long-term care, with solid knowledge of state and federal guidelines and strong leadership and

• Licensed Social Worker (LSW) or Master of Social Work (MSW) licensure.
• 2+ years of supervisory/management experience in a Long-Term Care setting.
