Responsibilities

• Conducts initial assessments of social, emotional, and physical needs; interviews patients and families.
• Develops and implements patient management plans with care coordination and referrals.
• Maintains records and documents case activity in accordance with policies.
• Participates in monitoring the quality and appropriateness of patient care.
• Provides supportive and crisis counseling to patients and families as needed.
