Job summary

Telecare Corporation seeks a Social Work Clinician II in Fontana, CA, under supervision to deliver clinical and casework services and to develop person-centered recovery plans in coordination with other services and agencies.

The on-call role offers a wage range of $31.33–$38.70 per hour, with geographic differentials. A Master's in Social Work and licensure progress are required, along with CPR, CPI, and First Aid certifications.

Qualifications

• Master's Degree in Social Work required.
• One year of direct service experience with individuals with severe mental illness required.
• Criminal justice clearance and education verification required.
• Must be at least 18 years old.
• CPR, CPI, and First Aid certifications on date of employment or within 60 days.
• Post-offer physical, TB testing, background checks, and driving clearance may be required.
• Valid driver's license and vehicle insurance with you as driver.
• Willingness to use company/personal vehicle for appointments with mileage reimbursement.

Responsibilities

• Provide clinical and casework services under supervision of a licensed clinician.
• Develop person-centered recovery planning.
• Collaborate with other services and agencies to support clients.
